1. APPLICATION FORM UNDER ARTICLES 11 AND 13 OF LAW NO. 6698 ON THE PROTECTION OF PERSONAL DATA
Under Law No. 6698 on the Protection of Personal Data, individuals defined as data subjects (hereinafter referred to as the “Applicant”) are granted certain rights under Article 11 of the Law in relation to the processing of their personal data.
Pursuant to the first paragraph of Article 13 of the Law, applications concerning these rights must be submitted to our Company, acting as the data controller, in writing or by other methods determined by the Personal Data Protection Board.
Your application and the request contained therein will be processed by the Company, as data controller, free of charge as soon as possible and no later than thirty days, depending on the nature of the request. However, if the processing of the request requires an additional cost, a fee may be charged in accordance with the tariff determined by the Board. Your request will either be accepted or rejected with reasons within this period, and the response will be provided to you in writing or electronically.

This application form has been prepared to identify your relationship with our Company, determine any personal data relating to you that is processed by our Company, and enable us to respond to your application accurately and within the statutory period. To prevent legal risks arising from unlawful or unauthorised disclosure of personal data and, in particular, to ensure the security of your personal data, our Company reserves the right to request additional documents and information for identity and authority verification, such as a copy of an identity card or driving licence. If the information provided in connection with your requests is inaccurate or out of date, or if an unauthorised application is submitted, our Company accepts no responsibility for requests arising from such inaccurate information or unauthorised applications.

Pursuant to Articles 11 and 13 of Law No. 6698 on the Protection of Personal Data, I request that the application set out below be evaluated by the data controller and that I be informed of the outcome.
As the Applicant, I acknowledge and declare that I have read the General Explanations and Legal Notices contained in this Application Form, that I am aware of my rights and obligations under Law No. 6698 and this Application Form, and that all information I have provided in this Application Form is accurate and complete. I request that I be informed within the applicable statutory period in response to my request.
I consent to the processing by the Company of the information and documents I have provided in this Application Form solely for the purposes of evaluating and responding to my application, delivering the response to me, and verifying my identity and address.
Note: All actions carried out in relation to your personal data under Law No. 6698, including deletion, destruction and anonymisation requested by you, are recorded. Without prejudice to other applicable legal obligations, these records are retained for at least three years in accordance with Law No. 6698.
